| La Cruz Azul, offer to its insured's groups the Organ and Tissue Transplant coverage, with different deductible options. This coverage is available to active employees, his spouse or husband and his eligible direct dependents younger than sixty-five (65) years of age, who life permanently in Puerto Rico, its territories and dependencies. |
| The transplant procedures are subject to La Cruz Azul preauthorization and should be realized in Puerto Rico or United States, through the providers network contracted by International Managed Care Services (IMCS). IMCS is a native corporation with national experience in the management of health care providers network. |
| Services are covered one hundred percent (100%) of the usual, reasonable and customary charges, subject to a maximum benefit of one million dollars ($1,000,000) per insured per policy year, up to a maximum of two millions dollars($2,000,000) lifetime, and is subject to the established limit per transplant. |
