  2. Actual rates
  3. Current insurance carrier
  4. Employer contribution
For Groups with 10 or More Employees
  1. Group Name
  2. Actual rates
  3. Current insurance carrier
  4. Employer contribution
  5. Benefit summary that includes deductibles and coinsurance (current and proposed plan design)
  6. Census that contains employee name or sex, date of birth or age, and type of contract
  7. Utilization or experience is useful but not required
For Large Groups with 75 or More Employees
  1. Group Name
  2. Actual rates
  3. Current insurance carrier
  4. Employer contribution
  5. Benefits
  6. Experience is required (at least 2 years)
  7. Shock claims
  8. Commission requested
  9. Deadline for submission
  10. Employee participation in coverages
