| We have a variety of pooled product designs for group business ranging from 2 to 74 employees. |

| Our core medical plan is composed of a base plan and a major medical offering. There are approximately 5 base plans and 5 major medical options that can be offered to our pooled business in the range for 2-74 employees. |

| Similar to the medical offering we have a large array (i.e. 11 plans) of drug coverages also available. These plans vary in copays, deductibles, formulary and maximums. |

| We currently offer 4 dental plans to our pooled group, that vary by classes covered. Our offerings include diagnostic, preventive, restorative, periodontal, endodontic, and orthodontia services. |

| For groups greater than 74 employees we can customize our benefit offering for all coverages based on the necessity and requirements of our prospect clients. Our sales representatives can be very useful in working with you to tailor plans that provide for cost containment and ample coverage at the most affordable price/cost. |

| We offer the following funding alternatives depending on the size of the prospective group: |
- Fully insured pool
- Fully insured experience rated
- Demographic rated/guaranteed cost (i.e. groups 10 emps and above)
- Minimum Premium
- Cost Plus
